Is this a .docx import template?
If so, you might try re-importing it & setting up the <tag> mappings again.
(use a new template name so you're not overwriting the old one)
A post yesterday seems to suggest that the mapping names might have been changed.
(obviously, this shouldn't have broken your template - but, ours is not to reason why!)
