I am enthralled with this thread, it just dumbfounds me how something that would take less than 5 minutes to correct keeps getting the most unhelpful replies to customer requests. 

Thanks so much for describing the multiple steps it takes to access the form that Quickbooks requires we have on file. Why in the world any of us would think we should be able to access it from the link provided by Quickbooks is insanity.

You actually went to the site, found the PDF and posted a SCREENSHOT of it. Why wouldn't you just link the actual PDF to the post? 
and then have the nerve to send a link marketing Quickbook payments.
