Thanks for following along and sharing your concerns in the QuickBooks Community. 

I appreciate you taking the time to follow the steps that my colleagues have shared above. Updating is the only way of getting the the 3 page form. If updating doesn't seem to work for you, I recommend uninstalling and reinstalling QuickBooks. This way, it acts as a fresh up to date install. 

Before you uninstall QuickBooks, be sure to do the following:

• Download the installer for your version of QuickBooks.
• Have your license number handy. It should be on the original package or in the purchase confirmation email if you bought a digital version.
• Write down your product year and version.

Once you've done those steps, you're ready to uninstall and reinstall. You can see our guide for additional information about this process in Clean install QuickBooks Desktop.
