I know the feeling when you are asked for a code and CAPTCHA every time you log in. Allow me to join this thread so I can help you in fixing this.
Please note that QuickBooks Online has two security features in addition to the password. Those are multi-factor authentication (MFA) or two-step verification. Unlike MFA, you'll be asked for a code every time you log in if the two-step verification is turned on. Also, we can't turn off the MFA so you'll still be asked for a code even if the two-step verification is turned off. Also, clearing the browser's cookies and cache can help fix any issues in QuickBooks Online. However, it can trigger the MFA. This is because cookies are files that save information about your browsing session. For example, this file saves the info when you check the "Remember my user ID" box when logging in, and clearing them means you'll need a fresh start.
Another thing that can trigger MFA aside from clearing the browser's cookies and the cache is when you use a new browser or new device.
If this behavior persists, I suggest you reach out to our Support Team again. They can create a ticket about this for investigation.
With regard to the bank, please note that they have their own security settings. We need to complete it since they require it unless they offer an option to disable it. You can reach out to your bank to check.
For the IP address, I suggest you seek help from your IT personnel. It could be because of your anti-virus program. An IT person can further check and fix it.
