You can follow the steps to update QuickBooks to the latest release, then run the Verify and Rebuild tools shared by my peers above. 

If you already did and the issues persist, I'd recommend contacting our Technical Support team to help you check and identify what's causing this to happen. That way, they can also submit a request to investigate the issue if necessary.
