I've replicated this on my account and see two different forms. Here's the screenshot for your visual guide.

Let's open these two forms on your account and check if one of the forms shows the CA DE9 details. If both forms are showing the CA DE9C information, you can perform some troubleshooting steps. 

 

Let's start by logging in to your QuickBooks account using a private browser. Then, check those two forms again.

 

If you see the CA DE9 and CA DE9C, you can go back to the regular browser. Then, clear its cache and cookies to refresh the data on your company file.

 

Follow the steps to clear the cache for your browser.

 

After clearing the cache, make sure to restart your browser for changes to take effect. As an alternative, you can also use a supported, up-to-date browser.

 

After that, log into your QuickBooks account and check again the CA forms. If you're getting the same results, I recommend contacting our Live Support Team. They can create a case to investigate your account so you have the correct forms ready before the due date.
