Let me share some information about your concern.
You can ask for the employee's completed W-4 form. The IRS does require employers to keep the W-4 in their records for at least 4 years.
Later on, if an employee needs to change the information, they must fill out a new form, according to the Internal Revenue Service (IRS). This is why employers should keep copies of the most current W-4s on hand.
